Wrinkle injection with hyaluronic acid (filler)

In medical terms injection with filler material is known as augmentation. Augmentation serves to eliminate a substance defect.

This can be:

  1. Wrinkles around the mouth
  2. Nasolabial folds (the folds between the nostrils and the corners of the mouth) which, for example, are very deep in certain basic illnesses and can give the face a hard expression.
  3. Mimetic folds which have become so deeply engraved over the years, for example, that initial treatment with botulinum toxin A does not immediately show the desired result.
  4. Sunken scars (e.g. acne scars, measles scars)
  5. Narrow lips
  6. Loss of subcutaneous fatty tissue (e.g. wrinkles on the back of the hands, creased décolleté, hollow cheeks, e.g. in the framework of a lipodystrophy syndrome)
  7. Congenital or acquired sunken areas or unevenness

The substances used are also referred to as fillers. We differentiate between liquid and solid fillers and amongst the liquid fillers, between absorbable and non-absorbable. Permanent fillers are rarely used nowadays.


There are three main reasons for this:

 

  1. Occasionally the human organism reacts to a certain substance as a foreign body and rejects it. The subsequent inflammation can lead to very unpleasant complications (e.g. the formation of granuolma).
  2. Unintentional overcorrection or mis-correction and side-effects remain for the rest of the patient’s life.
  3. Corrections made on a 34-year old face, for example, are usually no longer suitable at the age of 60 or 70.

At this point, at the latest, you would regret the treatment with permanent filler.


In addition to autologous fillers such as a patient’s own fat, we mainly use hyaluronic acid products. Hyaluronic acid is an important and natural component of connective tissue. In the skin, the concentration of hyaluronic acid, which is able to absorb a great deal of water, decreases dramatically with age. The connective tissue of the skin therefore stores less water – the skin appears less elastic and tends to wrinkle.


Hyaluronic acid which can be injected under the skin is prepared in such a way that it is not immediately flushed out again but is rather slowly broken down by the skin and eliminated via the liver. These products are cross linked hyaluronic acid products. These lead to a temporary increase in the viscoelasticity of the tissues and to improved nourishment of the upper layers of the skin by the subcutaneous layers.
